Clear Lens Exchange, also known as Refractive Lens Exchange (RLE), involves replacing the eye’s clear, natural lens with an artificial intraocular lens (IOL) to correct refractive errors such as presbyopia, hyperopia, or astigmatism. This advanced procedure sharpens both near and distance vision, often reducing or completely eliminating your need for reading glasses or bifocals. One significant advantage is that, if you have not yet developed a cataract, you will not develop one in the future after lens replacement.

Although lens replacement surgery for refractive purposes is not formally FDA-approved, experienced ophthalmologists may perform RLE off-label when it is clinically justified. Off-label use is legal and can be the preferred approach for some vision correction cases not suitable for LASIK or PRK—especially for individuals with presbyopia or extreme farsightedness.

Presbyopia is an age-related change that typically develops after age 40, resulting in a gradual loss of the eye’s ability to focus on near objects. For patients with significant presbyopia and moderate to severe hyperopia, Clear Lens Exchange may be the most effective option for achieving clear, glasses-free vision.

In select circumstances, RLE may also address myopia (nearsightedness), but LASIK or PRK are generally considered first if appropriate for the patient’s corneal health and visual needs. For individuals over age 55, especially those with challenging refractive errors, Clear Lens Exchange can provide a lasting solution when laser vision correction is not advisable.

The surgical process for Clear Lens Exchange closely parallels cataract surgery. During the procedure, a custom IOL is selected to match your specific vision profile and ocular health. Rochester Eye & Laser Center offers multiple IOL options:

Monofocal IOLs:

Designed to provide clear vision at a single focal point—either distance, intermediate, or near. Glasses may still be necessary for additional ranges if multifocal correction is required.

Toric IOLs:

Engineered specifically to correct existing astigmatism, with the goal of minimizing or eliminating the need for distance glasses.

Multifocal or Extended Depth-of-Focus IOLs:

Allow for vision at multiple distances, significantly decreasing dependence on glasses for most daily activities.

Light Adjustable Lens:

The first and only lens that can be customized after surgery, giving you the opportunity to Test Drive Your Vision! This lens allows you to have the clearly better vision you are after.
